- Original birth certificate
- Shot record
- Address of school they attended
- One (1) proof of address (power, water, gas)
- NO phone, cable, insurance or credit card statements. NO EXCEPTIONS!
- Parents name must match the name on the bills
- Parent photo ID
- Copy IEP if Applicable
In District Transfer:
- Withdrawal Paper
- One (1) proof of address (power, water, gas)
